During this time a medical provider will:

ā Review health concerns and wellness goals
ā Assess current lifestyle factors such as nutrition and physical health
ā Provide professional recommendations for services that best meet the employee's needs
ā Coordinate care recommendations when appropriate

How to Access Services Contact Foundations EAP directly
Your EAP includes a predetermined number of mental health sessions and life management services. To find out the specific number of sessions available to you, please refer to your benefits booklet or contact us directly.
EAP Intake
Our Intake Process
Our intake process is easy, hassle free and is completed in 4 easy steps:

1. A referral for services is emailed to our office.
2 Our staff then contacts the client directly to schedule their Intake Session.
3 Clients are given access to their "Client Portal". Using this portal, clients will access important consent documents that must be signed/completed prior to their first session.
4 An Intake Session is conducted, and a treatment plan is developed, in which specific goals are outlined.
EAP Referrals

How are Employees Referred For EAP Services?
Self Referrals: An employee can contact their EAP representative directly to schedule a session for services. Upon scheduling, the employee is given the opportunity to either meet with their clinician onsite in person OR via our confidential, HIPAA compliant telehealth/virtual platform from the comfort of their own home or office.
Official Referral: If a manager or human resources professional becomes aware that an employee is struggling, they can make an official referral. If this type of referral is made, a manager or human resource professional should make sure that the employee is aware that he/she is being referred for services and is aware of the benefits of receiving such services, which includes helping the employee to resolve problems impacting their performance or mood at work.
